Form with Check Boxes

I'm trying to create a workflow to allow users to search for parks by amenities.  I'd like to have a form with check boxes representing each amenity and to query the parks based on those checked.  I can't find an example of anything similar.  I'd appreciate any help on how to get started or sample workflows.  Thanks!
