Widget 'SelectionStar' was not found / The requested operation failed. Try again later, or inform your application administrator of the problem.
I'm setting up a map in my viewer where there is only one feature showing. The idea is the user will click on them and then in the resulting map tip, I will have a link there that fires a workflow on that feature.
Every other time I click on a feature (litterally every other time) I get the "The requested operation failed..." error message in Alert window. In the pull down log, I see another error stating that "Widget 'SelectionStar' was not found" - I have no idea what that widget is or where it should be... presumably it's someting ootb. That widget error repeats 3 times in the pull down log, then finally a final error: "Error performing identify operation on service <service>. Error details:" the details get cut off in the pull down so I hit CTRL + A to copy all of the text from the pulldown and pasted it into a notepad... Error details are empty there. I checked the System and Access logs and there is nothing related in there either.
Why does this work only 50% of the time? Also, map tips / clicking aside, if I pan the map, the features disappear... pan a second time they show up again.. pan a third time and they're gone... pan again they come back.
This is very strange and I'm not sure where this is stemming from.

I noticed I was getting some errors in the ArcGIS Server logs so I restarted the AGS windows service and that seems to have improved the behavior. I no longer get the service error, and the visibility issue also corrected.
I'm still seeing the 'SelectionStar' widget error though in the pull down log.0 -
I've never heard of this widget, are you positive that its OOTB? 0 -
I have never developed a widget. I asked around with our other developers and they say they never did either but are verifying against our code base. I've never seen this before either. 0 -

Selection Star is a new feature of the 2.12 viewer, it mimics the star function the Silverlight viewer had. Please make sure your versions match up (2.12 with 4.11) and maybe try redeploying the viewer to IIS (Post Installer option). Failing that, feel free to enter a support case.
