Creating Features/Set Feature Attribute activities over writing each other

I'm having issues creating multiple deficiency features in a workflow being accessed by the HTML5 Viewer.

I have a get layer activity which returns me the attribution schema from an ArcGIS REST Service, I'm using the ".layer.templates[0].prototype.attributes" property from the get layer activity to automatically assign this attribute schema to a new feature using the create feature activity in WF5 (create 1 feature per possible deficiency).

When I record more than 1 deficiency on the form, it seems that all of the deficiencies that I create using separate create feature activities all have the information from the last deficiency I entered on the form and when I check the ".layer.templates[0].prototype.attributes" property, it has the same information as well.

Does the create feature activity only create a link to the property that is referenced in the activity or is it supposed to create a copy of the object that was reference into whole new object that functions on it's own?
