Inconsistent/Wrong Results

I have a workflow that creates a buffer, collects data, and generates a report.

During testing, every several generations randomly results in incorrect output. Like, really wrong. It just seems to be picking some other location on the map, and collecting a weird mix of data to return.

How can I improve reliability?
