Hi Andy,
You can specify which scales to print at in the Print Templates section of Rest Manager. Click on Edit beside your print template, and go to the Map Scales tab. Here you can enter the different scales that you would like your users to be able to print at.
Beside the Map Scales tab, there is a Text Fields tab. If your print template contains at least one text box control, then when you select Add Text Field, the text box you have added to your site will show up in the Data Field Name dropdown box. The name you see here is configured in the Data Field property of the text box in the rpx file. Here you can specify a display name and a default value, and make sure that the Editable checkbox is checked if you would like your users to be able to edit the text when they run the print template.
Now when you select Create a Printable Map from the I Want To menu or the Toolbar, you will be able to select the scale that you want to display the map at, as well as the text for the title of the map.
