in the print template, if you do not specify a dpi is the default 96 dpi>
if the print template, if you do not specify a output format is the default pdf?
Can the defaults for print templates be set in the sites viewer.xml?

You can make the changes in the Site.xml, or you can use GE Manager to make the changes. I like making the changes in the xml because I have multiple print templates with multiple settings and am too lazy to manually enter them for each template. Search for the <PrintTemplates> section of the Site.xml to make changes to your print template settings.
<SupportedMapScales> for preset scales
<SupportedOutputFormats> for output formats
<SuportedResolutions> for dpi levels
