Display Form 'Required Value' is enabled but still able to Submit Form

I have a Display Form activity configured in my workflow, and each field in the form has a 'Required Value' validator enabled. When running the form in my web map, I am still able to submit the form without inputting any information. Am I missing something?

  You should check the "Causes Validation" checkbox on your submit button.
