Overview map not showing up on print template

We have just discovered that the overview map is no longer showing up on the printed map.  The area for the overview map is now a gray box.    We have recently upgraded to the latest versions and also loaded a new map service in manager.  The overview map in the application works great.  Any ideas why it disappeared from the map template.
