error applying feature edits html 2.2

I am in the process of moving our map service to html viewer. 

 

I have a feature that my users edit in silverlight - create, edit attributes, delete. It all works fine.

 

In html viewer I get an error when I try to "create feature" that says "there was an error applying feature edits". My workflow for this feature creation also fails.

 

I cannot figure out the problem as it all works fine in silverlight in the same map service.

  If anyone encounters this problem - I resolved it by adding the feature again to the map service.

  Sylvia, what do you mean by adding it again to the map service?

  I removed it from the map service and added it again
