File attachment failed. Not Found

I am trying to add attachments to a Feature Service with attachments enabled and am getting the Alert: "File attachment failed. Not Found." I saw some of the other attachment errors reported in other posts but none with the Not Found error. Any insight would be very helpful.

  Hi,

   

  If this issue is still outstanding then I'd compare your service to the sample service below which supports attachments. This service works as expect in GVH 2.10.x, see the screenshot below with my attachment appearing. 

   

  I'd also check the Chrome browser tools for any console errors or the network tab for any HTTP errors.

  Fixed mine by removing the proxy.ashx? from the Proxy URI field in the Viewer>Application setting

  Thanks Richard, that is what I ended up doing as well, I had kind of forgotten I had this post and forgot to add my solution.
