Hi Jake,
Would it be possible for you to share your server's URL so that we can try to reproduce this issue locally? What version of ArcGIS Server are you using?
You may be able to see an error in the console for Chrome Developer Tools (F12) when trying to add the data source.
Thanks, Stefan

Hey Jake,
Can you confirm the following for me:
- That you're running an on-premises instance on Server 2K16
- The ArcGIS Portal version and deployment you're using, ArcGIS Online, Portal for ArcGIS 10.5.1, etc
- The URL to your portal
- The level of your ArcGIS User performing the failing action (Level 1 or Level 2)
Regards,
Paul

Hey Jake,
That's interesting, I would have suspected it to be a permissions issue.
Mind capturing a HAR log for me for a session loading the designer, signing in, start creating a new data source, test it, and then attempt to save it (which will fail presumably). Instructions here: https://www.inflectra.com/support/knowledgebase/kb254.aspx
For privacy reasons, please email the captured log to my email - phunter@latitudegeo.com

Hey Jake,
Thanks for sending that over, apologies I didn't reply to your post. There was a small issue we found and have fixed for the upcoming 5.0 release.
Thank you for reporting the issue and capturing the HAR log for me.
