Geoprocessor example?

Anyone have a Geoprocessor activity in a workflow they can share as an example? I am not sure what I am doing wrong. I have tried many options, with and without parameters on different GP services, etc. No luck. Errors below, but nothing obvious as to what is failing. This service in example runs just fine @ the REST endpoint side.

  Thanks Stefan. Turns out the workflow and service works as long as I am using the async GP activity, not the synchronous one - did not realize this. This was not really a factor in WF4 from what I recall.
