    I am experiencing the same problem and haven't found a solution yet.

    The issue here might be the visible scale settings in the MXD.  Try changing the minimum scales of the layers that you are using in the overview map so that those layers are visible at a large extent, or simply show those layers at all scales.  Save the MXD (and MSD, if you are using one), refresh the map service, and clear the REST cache.

    I'll certainly check into that, but what I'm confused about it they were printing when I designed/deployed them.

    I'm sure I had the same problem as you Charles.  It's exactly what I was experiencing.  It worked for awhile in the printed maps and then for some reason wasn't working anymore, though it did on screen.

    I deleted my map services in ArcGIS Server and served them again, using the same files.  Once I added them to my application again they worked fine.  So while I don't have an answer for why, perhaps you should just try serving them again.
