Specify a default value for print template combo boxes

I'd like to be able to specify default values for the Select Layout, Output Format, Resolution and Map Scale combo boxes when printing a map.  For example, for Resolution, I'd like it to default to our 192 dpi setting, not the lower 96 dpi setting.
