Data entry form - Workflow

I have set up 'Configuring Editing Tools for Feature Layers' for a point layer but I am unsure how to link this editable layer to a data entry form. I would like users to be able to add a point and fill out a data entry form for that point (including drop downs and checkboxes). I also need users to add photos and videos for that point. How do I get started with this? After I have configured a feature layer for editing, what are the next steps to add a custom data entry form to an editable point layer?
