This is almost exactly what I'm looking to do as well.  I'm hoping the Ad Hoc query builder is similar in concept to this!

I believe that a query builder is going to be a standard module in one of the upcoming viewer releases.

A Query Builder is in the works for the Silverlight Viewer.

I'm not sure why their site reverts to the secured site, but you can access the public site from:

    http://countyofrenfrew.geocortex.com/Geocortex/Essentials/RenfrewCounty/REST/RenfrewMapViewer.aspx?

This shows a dropdown of all of their apps - just select the public one.

    @ Ryan

will we need a workflow to make this work?

It will not use workflow. It will be a new module for the Silverlight Viewer.

Glad to hear that it will be incorporated as a module.  However, it would still be very helpful to get a workflow like that one posted in the Gallery.  It has so many components that a person could use for building other workflows.  It would be a great learning tool for understanding how forms work with different user interaction.  I understand that Geocortex built that workflow for Renfrew County, so if its possible to add it to the Gallery, we'd greatly appreciate it.

    +1 on that one Sonia.

With little in the way of examples in Workflow the more that LG can post up there, not matter how simple or hard at least gives us some great examples to leverage off.

I agree with you Gareth.  Workflow sounded quite simple when it was released- line up a series of tasks, configure their parameters and voila!  Unfortunately, I've found that workflow is far more demanding than that.  Most often it's my unfamiliarity with what processes need to go together to accomplish a task.

I've found that studying the ESRI Silverlight API helps to some extent, but for most people (myself included), seeing examples shows us far more.
