Add Field to Cascading List

I've modified the Geocortex example of find schools in a certain zip location, and now have it that you can pick a address type category and then the amenities within that location. After the workflow reads the names associated with that category, is there any way to add addtional fields to when the second pick list is displayed?

/customer/servlet/servlet.FileDownload?file=00P6000000eltieEAA

Since it is possible to have names that are quite similar, I'd like to add an address beside the name to differentiate them.

  I also would like to know if it is possible to add additioanl field(i.e,textbox) on displayform based on some condition. I know you can assign a value to an exisiting item(i.e.,groupbox description) on displayform using runtimeModifications, but wonder if addtional field can be added to the displayform.
