Startup workflow for HTML5 Ver. 2.3?

I am trying to use the StartupQueryWorkflow_Revised in the Code Gallery to zoom to a parcel with the HTML5 viewer, but it seems to ignore the run parameters. I have hardcoded queryLayerMapServiceId and queryLayerName values in the Workflow Create/Edit Wizard in Essentials Manager (ID = findParcel, queryLayerMapServiceId = 21 and queryLayerName = Parcels) and just pass the query parameter via the URL, but have tried to pass those parameters via URL as well and get the same result: stays at the initial extent. I am using a reverse proxy which seems to work fine, here is the URL: http://www.adacountyassessor.org/adamaps/?run=findParcel&parcel%3D'R1625503621'&LayerTheme=AerialsOn or http://www.adacountyassessor.org/adamaps/?run =findParcel&queryLayerMapServiceId=21&queryLayerName=Parcels&parcel%3D'R1625503621'&LayerTheme=AerialsOn (I have forced the default viewerConfig in the Index.html to this single site for external users)

LayerTheme parameter works fine, any suggestions?
