Duplicating Records on Reports

I have workflow that populates a template report. It is repeating the dataset exactly the same number of records returned from the query. So, if the query returns 16 records; the table repeats itself 16 times in PDF output. Here's an example:

/customer/servlet/servlet.FileDownload?file=00P6000000em1SKEAY

I've put Alerts to confirm the number of records returned from the query and the row count from the data table variable. They are all returning the appropriate values. I am using a subreport to load the data into the main report.

How can I stop the repeating of the data?
