Hi Chris, did you happen to find a solution?  I am receiving a similar error.

    What versions are you using?

Also, do you have your services URL in the proxy.config?

I am using Essentials 4.4.3 and Viewer for HTML5 2.5.1.

And, yes, I double checked and my services URL is in the proxy.config.

We had issues using earlier versions, but adding attachments works fine for us now.

What does Fiddler show is happening when you try to add the attachments? Can you add attachments using the featureservice in ArcGIS Online?

For what it’s worth. I had this exact same issue from an Android tablet. The only way I could fix it was to remove the proxy URI information. I found the solution on a different post. Unfortunately, it’s been so long I do not remember who posted it.

    Hi,

If you are getting a "File attachment failed. Forbidden" error when trying to add attachments to a feature in the HTML5 viewer, then please go to your proxy.config file located in the wwwroot folder and add the feature layer URL to the list of URLs. Make sure you include the layer ID to the end of the URL as well so it should look something like this:  <serverUrl url="http://domain.com:6080/arcgis/rest/services/LayerName/FeatureServer/0" matchAll="true"></serverUrl>

    That should fix the issue.

    I hope this helps.

    Carmen
