Geocortex Identity Server asks for wrong credentials

We have both windows integrated and geocortex identity server security enabled in our production and development environments. It works well in development. However in production, when you are asked to choose Geoctx Identity Server or Windows Integrated, you are prompted to enter some kind of windows-based credentials which will not work. Neither will credentials we have created for Geocortex ID server (we have a user created who has access to the map).
The two images show the production version (not working correctly) and the development version (working correctly) - this is what happens on both sites after clicking 'Gecortex Identity Server' to log in to the map. Any thoughts on where/how to configure the production environment correctly?
Incorrect (production):

Poking around IIS, noticed a discrepancy in the Application Pools between our dev and prod servers. Looks like it's best to have the IdentityServerAppPool use the ApplicationPoolIdentity. We had it set to something else on the server. This change fixed the issue.
