Photo Attachments - Invalid

I have been beating my head against the wall on this one.  I have a Featureclass with feature attachments set up in an ORACLE database.  This is then published as a secure feature service using ArcGIS Server 10.2.1.  

I have no problem adding, deleting, displaying the attachments if i am using ArcGIS Desktop.

I also have no problem viewing or deleting attachments using the HTML5 viewer or using the attachment manager workflow. The problem occurs when adding an attachment.  Everything appears to work correctly as a record is added to the ATTACHMENT table.  I  cannot view any attachment added using the web client even using ArcGIS desktop.  They appear there but nothing happens when I double-click the image.  If I try to view the image in the HTML5 Viewer I get "Error: Invalid URL, Code: 400". 

I am using GE 4.3.1 HTML5Viewer 2.4.1

Any Ideas what I am doing wrong?
