HTML 5 Viewer - Can't query any layers

After upgrading, query tool says "This layer cannot be queried." for every layer even though query is enabled for the service.  How can this be fixed?

  This happened to me also when I upgraded to 2.5. I ended up removing the map service and added it again. Then it started to work. But just today, the problem came back. I did not upgrade or anything like that. It seems some configs are causing this issue, but I am not sure what. Only thing I can remember doing is to re-rearranging some tools in the toolbar.

  Did anyone find an answer to this.  I have the same problem.  The query tool works great in one of my applications and not at all in another despite having "allow Query Operations" checked off for the layer
