template report error

I'm trying to create a simple workflow that selects fields from a parcel layer and creates a simple report. I've captured the parcel data and added it to a table. The workflow errors out in the step with a Template Report control with the following error:

 

Exception has been thrown by the target of an invocation.

 

Control name doesn't exist or invalid

 

Parameter name: name

 

I've provided the Template Report control with values for the Template Uri, DataSet, Output Format, and Report data fields.

 

Can anyone decipher this error message for me?
