send web request activity in Workflow 5

Is there any example for setting the various parameters for the "Send Web Request" activity in Workflow 5? The examples I found were in earlier workflows and not applicable. I'm trying to post a formData message to a geoprocessing service's upload operation and will need to set the header and other properties to the correct type. Thanks.
