Form Designer Input Geometry

When passing in an input geometry into the form designer (it's a valid geometry and I can report on the coordinates) none of the combo-boxes load data when I click off the Filter By Geometry box.

 

The comboboxes load properly when not using geometry, but when using it it shows the "loading wheel" endlessly.  I can confirm that there should be overlapping geometries.

 

Has anyone else see this type of behaviour before?
