Is it possible to customize labels to include data from two fields?

  Yes, if you do it in ArcMap before publishing. The generalized workflow would be: Open the Properties window (in ArcMap) for the desired layer; choose the Labels tab; build the Expression containing as many fields as desired, separators such as '-', '\', etc.; DO turn on feature labelling in ArcMap; publish the service; in Geocortex the default label will come in as built with multiple fields; you can then decide if the labels should be on or off by default.
