Get a collection of attributes from a layer

I have a layer from the Get Layer activity, I would like to get the attributes of the layer an present them in a drop down list in a form.

Does anybody know how you do this please, I guess I need to form a collection that I can use in the Set Form Element Items, I think I need to use Get Form Element Items... but I do not know what I need to use in the activity

  Here is an example (https://www.arcgis.com/home/item.html?id=801d862a7b804156a65fd907b2505c9e). We read a JSON file and then populate radio buttons dynamically. Specifically, look in the subworkflow for each of the Radio Buttons in the Display Form. Unfortunately, this won't run because we are running a custom Form Activity but the logic is the same if I understand what you are trying to do.

  Trying to stay safe, lock down restrictions are now being lifted slowly so fingers crossed
